Asthma Day

The Bronx-Lebanon Family Medicine Department organized the first

Asthma Day clinic in Bronx-Lebanon Hospital at Ogden on

January 4th, 2012.

Preparing our staff.  We began by conducting a staff meeting to review the goals for and importance of high-quality asthma care.  We also highlighted the importance of each individual staff member in providing quality asthma care.

Patients would be asked about night symptoms, use of short-acting agents, any symptoms with activity, and lost days of work or school.  Residents would administer the Asthma Control test, and the Nursing staff would assess vital signs, peak flow, and pulse oxicemetry.  The Community Health Workers were assigned the task of patient education on Asthma and the use of medications. 

Scheduling Patients for the Asthma Day.  We identified Asthma patients from our Asthma registry.  Our Registrar called the patients and made appointments for them.  A reminder letter was also sent in the mail.

The Asthma Day.  The patients who participated felt the visits were very useful.  Our Staff- Physicians and Nurses also found this approach very useful.  Since it allowed them to focus mostly on one condition the entire morning, they felt their ability to provide consistently good patient care and education was improved.

Results.  We are continuing to fine-tune this program as we learn by experience. We will continue to do more Asthma Planned visits in the future.  We also plan to develop similar pilot programs for other chronic conditions, such as Diabetes and Hypertension.

The Diabetes Day clinic is scheduled for March 21st, 2012
