BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, BronxWorks feeds, shelters, teaches, and supports our neighbors to build a stronger community. Bronx Lebanon Family Medicine is proud to partner with BronxWorks to reach out and care for and connect with our community.

Learn It, Grow It, Eat It
The Community Health Worker Program is happy to have joined with Learn it, Grow it, Eat it in order to help educate the community about the importance of eating healthy.  Learn it, Grow it, Eat it is an organization for teens to grow their own nutritious fruits and vegetables.  They currently work with over 400 teenagers and, along with Family Medicine, are educating our community about why eating well is important, as well as giving the community an affordable, self-sustainable and educational way of doing this.

Rocking The Boat

The Community Health Worker Program has been collaborating with the community based organization Rocking the Boat. The CHW’s have provided health education classes, participated in career days and contributed financially to Rocking the Boat as well as attended their annual fundraiser.
Rocking the Boat uses boats to help young people challenged by severe economic, educational, and social disadvantage into empowered and responsible adults. Participating during the school day, after school, and throughout the summer in hands on wooden boat building and on water education programs, young people in the South Bronx develop the self confidence to set ambitious goals and the skills needed to achieve them. The organization serves over 2,700 young people and community members annually. Kids don't just build boats at Rocking the Boat, boats build kids.

Siena House

The Sisters of St. Dominic, founded in 1879 to minister to homeless children, continue to respond to the needs of the homeless of our day. The Siena House was founded in 1990 as a temporary residence for single mothers and their children. Bronx Lebanon Family Medicine's Community Health Workers have joined with these Sisters to provide health care and health education and to assist the Sisters in their goal to empower the mothers and their children to take a more powerful role in the community.
